BOOKING INSTRUCTIONS
We offer private sessions by appointment only. We are not currently accepting new patients. If you would like to join our waitlist, please contact us.
Our patient portal allows you to view our calendar, book online, complete your intake forms, manage your appointments, and contact us using HIPAA compliant messaging. After your appointment is confirmed you will receive a confirmation email that contains instructions for the door entry code needed upon arrival, our location, parking information, and a link to complete your intake forms online via the patient portal.
We are in network with most major insurance plans. We do not accept Cigna, Kaiser, CareOregon, Trillium, or Medicare. If you would to use insurance please provide us with your insurance details via the patient portal after you book; or by calling our office. As a courtesy, we will verify your benefits to make sure you are eligible for coverage and relay your coverage details quoted by your insurance company to you via chat on the patient portal. > For our patients with health insurance
We are In-Network with most major insurance carriers. As a courtesy, we will verify your benefits before your first visit. If you would like us to check before you book, please text or email us a picture of the front and back of your insurance card, include your address and date of birth. We have a 24-hour cancellation policy so it is important that you determine your insurance benefits prior to this cancellation window to avoid incurring any fees related to non-coverage or a high deductible. Please remember your insurance policy is an agreement between you and the insurance company, and it is ultimately your responsibility to pay for any balance not paid or covered by insurance. We do not provide superbills for out-of-network billing. Please provide a copy of your insurance card and proof of identification at check-in.
> For our patients with car, bike, or workplace accidents
In addition to health insurance, we also file claims for Worker’s Compensation, Motor Vehicle Accident (MVA) including bicycle accidents, and Personal Injury Protection (PIP) claims. If you have been involved in an accident as a driver, passenger, pedestrian or cyclist, you have the right to see the medical providers of your choice, including acupuncturists. In the state of Oregon, it is mandatory that all drivers carry $15,000 of medical coverage to be used in your recovery. Even if the accident was your fault, you still have this coverage. Your bills are completely covered by your insurance company and you are not responsible for any out-of-pocket expenses.
> For our patients without health insurance
If you are not covered by your insurance, or plan to pay out of pocket, we apply a time of service discount for our patients who pay at the time of service. This is an all-inclusive service rate with no procedure or diagnostic codes.
> For our patients with economic hardships
We offer discounted rates for veterans and active service members, and patients with qualifying financial hardships.
> For our patients with Kaiser Permanente / CHP
Although we are not in network with Kaiser and cannot bill them for services, we are a member of Kaiser's ChooseHealthy program which provides discounts on a variety of complementary and alternative care resources. Beyond any alternative care plan benefit you may have, we offer you 25% off our standard rates when you make an appointment through the ChooseHealthy program. No doctor's referral is needed. Please note: The ChooseHealthy program is not insurance. It is offered in addition to your health plan coverage as an extra resource for you. The ChooseHealthy program provides for discounts from participating specialty health care providers. You are obligated to pay for all services from those providers, but will receive a discount from those participating providers for services included in the program.
> For our patients who are veterans with TriWest VA CCN
In order to access acupuncture benefits, the veteran must receive a referral from the VA to a community care provider. The veteran must advise the VA that Silver Lining Acupuncture / Dr. Erica Robyn Burley, DSOM, L.Ac is their preferred provider. After the VA issues the referral we will receive an authorization from TriWest. Once we receive the authorization for care, we will reach out to you to schedule if the VA CCN has not yet done so on your behalf.
> For our patients with medicaid / oregon health plan (OHP)
We accept the following Health Share (OHP) plans - Legacy Pacific Source, OHSU Health, Yamhill CCO, OHP Open Card and Providence Health Assurance. We do NOT accept OHP plans managed by Kaiser, CareOregon or Trillium. If you have OHP, please be sure to specify which CCO manages your plan (printed on your Health Share card). If you have Kaiser, CareOregon, or Trillium please do NOT book online unless you plan to pay out of pocket. If you would like to switch your plan to one we do accept or to a plan that does not require a PCP referral, contact Health Share to change to a plan managed by OHSU Health or Legacy Pacific Source. According to the Health Share website (https://www.healthshareoregon.org/members/my-health-plan) - “You can switch health plans if you want—just call us at 503-416-8090 or email us at info@healthshareoregon.org.”
Please be sure to review our cancellation policy. Cancellations with less than 24 hours notice and missed appointments are subject to a $50 fee.
