If you would like us to verify your insurance coverage, please provide your date of birth and member ID#.

Location
We are located in what is now called Portland, Oregon in Multnomah County, on the occupied ancestral land and unceded territory of the Confederated Tribes of Grand Ronde and the Confederated Tribes of the Siletz Indians, specifically the Cowlitz and Clackamas peoples, as well as the Multnomah, Wasco, Kathlamet, Bands of Chinook, Tualatin, Kalapuya, Molalla and many other Indigenous peoples whose homelands are along the Columbia River. Land acknowledgement does not serve as a replacement for returning land and resources to their stewardship and care.

Parking
Ample off street parking is available and our location is easily reached by bus, bike or light rail. Our Parking Kitty zone is 49752. We have an ADA accessible parking garage with 4 spots designated for 2 hour visitor parking and an area for secure bike parking.
ADA Accessibility
To access the elevator, please be sure to enter the building through the parking garage. There is 1 designated ADA accessible parking spot and a ramp that leads to the elevator. Our office is on the 1st floor. Please note the garage is closed on Sundays (opens from the inside only).

Our Office
We are located in a quiet and cozy office in a LEED certified building has ADA accessibility via the parking garage and double stall restrooms on each floor that are labeled as “Men’s” or “Women’s” that require a code to enter. Each treatment room has its own table with an adjustable warmers, infrared TDP heat lamp, and HEPA air filter with UV light to purify the air. Read more about our COVID-19 safety protocols here.
