INSURANCE
We are in-network with most major insurance companies and accept motor vehicle accident (MVA) and Worker’s Comp claims. We also accept some Oregon Health Plan (OHP) Medicaid plans— see full list below. We do not provide superbills for out-of-network billing. If you are unsure about your coverage, please contact us and we will happily confirm your benefits for you.

We accept most insurance plans. As a courtesy, we will verify and bill your primary and secondary insurance on your behalf. If you would like to use insurance to pay for your visit, please inquire about using your benefits 48 hours in advance of your appointment. We have a 24-hour cancellation policy so it is important that we are able to determine your insurance benefits prior to this cancellation window to avoid incurring any fees related to non-coverage or a high deductible. It is important to remember your insurance policy is an agreement between you and your insurance company, and it is ultimately your responsibility to understand your benefits and pay for any balance not paid or covered by insurance.

In order to access acupuncture benefits, the veteran must receive a referral from the VA to a community care provider. The veteran must advise the VA that Silver Lining Acupuncture / Dr. Erica Robyn Burley, DSOM, L.Ac is their preferred provider. After the VA issues the referral, we will receive the authorization from TriWest. Once we receive the authorization for care, we will reach out to you to schedule an appointment if the VA CCN has not already done so on your behalf.

We offer a 20% discount to uninsured patients paying at the time of service. This is an all-inclusive payment at time of service rate with no procedure or diagnostic codes.

Although we are NOT in network with Kaiser Permanente, we are a member of Kaiser's ChooseHealthy program. For Kaiser patients who pay out of pocket, we offer 25% off our Payment at Time of Service discounted rates. Discounts do not apply to services billed to insurance, herbs or other products.

If you have been involved in an accident as the driver, passenger, pedestrian or cyclist, you have the right to see the medical providers of your choice, including acupuncturists. In the state of Oregon, it is mandatory that all drivers carry $15,000 of medical coverage to be used in your recovery. Even if the accident was your fault, you still have this coverage. Your bills are completely covered by your insurance company and you are not responsible for any out-of-pocket expenses. Please contact us within 48 hours before your appointment with your insurance claim #, adjustor's name and phone number to request that we verify your benefits and the status of your claim before beginning treatment.
Good FaiTh Estimate Notice
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises
